Reference:            Invitation of Quotation for Low Vision Devices/Equipment  

Sindh Institute of Ophthalmology and Visual Sciences (SIOVS) is implementing a project Promoting Inclusive comprehensive eye care services by adopting peek solutions in Province of Sindh (peek project) By CBM-International.” SIOVS intends to receive quotations for Low Vision Devices/Equipment details are given below, for this you are requested to submit your quotation, stating rates on below mentioned required terms. Quote should reach to the office of Deputy Director, Ophthalmic Community Survives SIOVS, Office within 07 working days.

You are therefore requested to dispatch or submit your sealed Quotation address to Program Officer SIOVS office @ Eye Hospital near Journalist Colony, Hyderabad by hand or via Courier, with best rates for the Low Vision Devices/Equipment, latest by 09 October 2024.

Note:

1. Considering the risk of COVID-19 infection, the supplier can submit quotations through any one of the following methods.

a. Through email to [email protected], please mention RFQ detail in the email subject line.  Quotations must be submitted on supplier letterhead in the form of” pdf/jpg”.

b. By hand in the tender box available at our office address “SIOVS @ Eye Hospital, Journalist Colony, Hyderabad” between 9:00 am to 3:00 pm Monday to Saturday on or before the mentioned deadline. Please consider instructions relating to social distancing and protection while submitting the bids in person.

c. Through courier to our postal address mentioned above. SIOVS will not be responsible for any delay due to courier company.

2. Rate must be given in Pak Rupees including all applicable taxes (WHT).

3. Payment details of the supplier must be clearly stated on letterhead e.g. to whom cheque should be addressed. Payment will be made through cross cheque after the deduction of the applicable taxes as per Govt. regulations.

4. Lowest price will not be the sole criteria for selection of supplier(s). Quality & Quantity of Low Vision Devices/Equipment, Delivery time and previous experience will also be considered.

5. Supplier must not be engaged in any corrupt, fraudulent, collusive or coercive practices including but not limited to applying/bidding by multiple names/companies in question or permanently black listed companies.
