Job Description
Title: Case Management Officer
Reporting to: Project Manager
Duty Station: Head Office Islamabad
Unit: Programme

Summary of Key Functions
The Case Manager will be responsible for helping Persons with Specific Needs (PSNs) including mentally or physically challenged individuals, illness, mental health issues or life-altering conditions to gain access to rehabilitation programs and other community services. This position reports to the Project Manager. His/her duty include meeting with PSNs/PWDs and their families, connecting them with professional services and maintaining consistent communication to assess their wellbeing and make changes to their care / service delivery as needed. The candidate will also be expected to establish and maintain contacts with the service providers.
Roles and Responsibilities
- Identification of individual cases with specific needs and those at risk-
- Manage, coach and orient the Community-based Protection Officers and Community Volunteers on case management.
- Coordinate and ensure that cases are managed according to guidelines and standards.
- Ensure that all case management related M&E tools are utilized, information collected, analyzed and actions taken to ensure quality service provision.
- Develop, execute and monitor case management action plans/safety/care plans for beneficiaries, developed by Case Workers;
- Ensure proper documentation of individualized cases and follow-up processes.
- Work in close coordination with Project Manager and Community-based Protection Officers on issues related to program and activities.
- Work in close coordination with Project Managers, Field Officers – Team Leader (Field) and CBP Officer and regularly update referral pathway in collaboration with Social Welfare Department, Health department, Education Department and other service providers.
- Participate and ensure participation in peer-to-peer or other capacity building initiatives implemented at the district level.
- Ensure participation in HO/Field level case management coordination meetings and case conferences.
- Engage with service providers including government and non-government, for the establishment of referral pathways.
- Organize individual weekly and monthly meetings with Project Manager, Field Officers and CBP Officers.
- Dynamic professional with experience in project planning, coordination, monitoring and evaluation of medium to large projects.
- Strong project management and reporting, organizational skills and keen attention to detail.
- Must be able to provide technical advice and support to the Project
- Ensure that all data related to referred cases, counseling processes, and success stories are collected, documented, analyzed and coordinated with M&E Officers, IMS Assistants on weekly, bi-monthly, monthly and quarterly basis.
- Compassionate and able to relate to PSNs on various needs
- Motivation to encourage individual cases and follow their care plans
- Strong verbal and written communication skills to explain to PSNs, family members, friends and professionals, the case and care plan and maintain good case records.
- Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from CBP Officer, OVs, Health Care and social workers
- Flexibility to change care plans if they are not getting the best results
- Organization to manage several different cases at once
- Computer literacy to maintain and manage case records
- Any other duties assigned by the supervisor.
