1. Introduction

 Indroduction :

Instructions to Bidders : 

  1. Quotations: Proposals must show unit prices, extensions, and total price where required. Quotations must be a fixed price, expressed in Pakistani Rupee (PKR).

Bidders shall fill in the price for each item in each option in below format. In case of any discrepancy between a unit price and an amount, the unit price will be taken as correct and the amount adjusted accordingly. For any item for which a unit price or amount is not shown, it will be assumed that the Bidders is not including that item in their proposal.

Proposals must remain valid for not less than sixty (60) calendar days after the offer deadline.

  1. Negotiations: Best-offer quotations are requested. Award will be made to a responsible supplier submitting the lowest price, technically compliant proposal. However, Human Appeal reserves the right to conduct negotiations and/or request clarifications prior to awarding a contract.
  1. Evaluation and Award: The award will be made to a responsible Bidders whose offer follows the RFQ instructions and meets the source/nationality requirements. The proposal will be evaluated based on its responsiveness to the technical & financial requirements.
  1. Typeof Contract: Human Appeal intends to award a Purchase Order (PO) for this activity. Human Appeal may award the procurement in full or in part to one Bidders, multiple Bidders, or no Bidders 
  1. Terms and Conditions:  This is a Request for Quotations only.  Issuance of this RFQ does not in any way obligate Human Appeal to award a purchase order, nor does it commit Human Appeal to pay for costs incurred in the preparation and submission of a proposal. Human Appeal may award the procurement in full or in part to one Bidders, multiple Bidders, or no Bidders. This solicitation is subject to Human Appeal standard terms and conditions.  Any resultant award or purchase order will be governed by these terms and conditions.    
  1. Human Appeal reserves the right to cancel the order at no cost if the promised delivery date(s) is/are not met. The Vendor agrees to deliver/perform all supplies/services set forth above and on any continuation sheet(s) attached hereto for the prices specified hereinabove in accordance with the General Terms & Conditions set forth herein.

Examples of day-care surgeries includes Dialysis, cataract surgery, Angiography, stitches due to accidents, etc. Specialized investigations covered are MRIs, CT-Scans, Thallium Scan, Bone-scan & Endoscopy etc.

1- The quoted premium should be valid for one complete year and shall be charged by the insurance company on pro-rata basis for new staff enrollments/additions to group life policy and subsequently refunded on pro-rata basis for the employees that may leave the Human Appeal.

2- All existing and pre-existing conditions of our group of employees and their respective dependents shall be covered without submission of health questionnaires as mentioned above. New additions of employees and dependents shall be considered on a case by case basis subject to health questionnaires.
