Aga Khan Health Service, Pakistan is looking for competent Registered Nurse/LHV in Chitral Region under Living Lab Project.
Job Responsibilities:
The Registered Nurse / LHV will be responsible to:
- To become familiar with community health services
- To provide home health services to participants involved in the project
- Undertake training as provided by project staff
- Provide high quality personalized planned holistic nursing care to all assigned patients
- Administer medication safely, collect biomedical samples, applying knowledge of drug and dosages, refers drug manual and/or pharmacy information as required, monitors patient responses to medication and communicate reportable findings promptly
- counseling and educating patients on health behaviors
- Demonstrate efficiency in use of electronic monitoring equipment
- To ensure updated medical records of patients visited.
- Demonstrate competence in patient assessment and care plan accordingly
- Perform any other task assigned by supervisor.
- Be able to follow and fill healthcare algorithms
- Perform comprehensive patient and environmental assessments related to home health needs
- Develop care plan in consultation with clinician and identify referral requirements
- To plan daily home visits of patients
- Coordinate and implement care delivery while anticipating patient care needs across the continuum, utilizing nursing process (assessment, diagnosis, planning, implementation, and evaluation)
Education, Experience & Competencies
- Minimum Diploma holder, bachelor’s degree in nursing (BScN), post-RN BScN will be preferred. LHV qualification will also be considered.
- 1-2 years of nursing experience will be preferred. Experience of working in field will be highly regarded
- Ability to assess, plan and manage patient care with expertise as per protocols
- Willingness to work with shifts
- Excellent interpersonal skills, including communication skills
- valid license from Pakistan Nursing Council.
