Job Description:

Position Objective and Specific Tasks:

A new Integrated Services through Mobile/Outreach Unit initiative is being launched to improve access to immunization, primary healthcare, maternal and child health, and nutrition services for urban, peri-urban, rural, underserved, mobile and vulnerable communities.

The Integrated Health Services Provider will support the mobile/outreach team in delivering an integrated package of basic health services, with a focus on Zero Dose (ZD), Under-Immunized (UI), missed/defaulter children, children under five, women and other vulnerable populations, in coordination with the Vaccinator, Social Mobilizer and local health authorities.

Key Responsibilities:

  • Provide basic integrated health, nutrition, maternal and child health services within the scope of professional qualification during mobile/outreach sessions.
  • Support identification and referral of ZD, UI, due, defaulter and missed children and link them with vaccination services.
  • Conduct basic nutrition screening and provide counselling on breastfeeding, complementary feeding, hygiene and healthy nutrition practices.
  • Provide basic maternal, newborn and child health counselling and facilitate referral of identified cases.
  • Prepare and implement monthly advance tour plans and support development/updating of social profiles of assigned areas, including mobile/migrant and underserved populations.
  • Coordinate with the Social Mobilizer to link demand-generation activities with planned mobile/outreach services.
  • Counsel parents and caregivers on immunization, nutrition, child health and appropriate follow-up/referral.
  • Maintain accurate records of beneficiaries, services, referrals, ZD/UI and defaulter children and report through NEIR/MIS/digital platforms on a daily basis.
  • Identify and report suspected AFP, measles, NNT and other priority diseases, and report AEFI according to relevant guidelines.
  • Follow infection prevention, safety, confidentiality and other applicable health protocols during field activities.
  • Participate in review meetings, supportive supervision and other project activities as required.
  • Perform any other relevant task assigned by the Project Coordinator.

Educational Requirements and Skills:

  • Nurse, LHV or PHC/Nutrition Worker with a relevant diploma, degree or professional certification from a recognized institution.
  • At least 1 year of relevant field experience in primary healthcare, immunization, nutrition, maternal and child health or public health.
  • Experience in immunization/Polio, community outreach or nutrition programs will be preferred.
  • Good communication, counselling and interpersonal skills.
  • Basic knowledge of immunization, nutrition, maternal and child health and community-based services.
  • Ability to use a smartphone and digital reporting applications.
  • Ability to work independently and as part of a mobile/outreach team in challenging field conditions.
  • Local resident of the assigned UC/area and knowledge of local languages will be an advantage.
